The Intake System provides a public-facing Personal Data Inventory (PDI) form inspired by the ACBC/BCC counseling model. Prospective counselees complete a multi-step wizard that captures comprehensive demographic, family, health, and spiritual background data — all before the first session.

Public Form URL

Each organization has a unique intake form URL:
https://app.adoraplan.com/counsel/intake/:slug
The :slug is your organization’s portal slug, configured in Settings → Portal.

Form Steps

The PDI wizard guides counselees through seven steps:

Spam Protection

The form includes two layers of automated spam prevention:
  • Honeypot field — A hidden field that bots fill but humans don’t see
  • Time challenge — Submissions faster than 3 seconds after page load are rejected

Submissions Queue

Submitted intake forms appear in the Submissions tab within Cases.

Review Workflow

View Submissions

Navigate to Cases → Submissions to see all pending intake forms.

Review Details

Click a submission to view all collected data organized by section.

Add Reviewer Notes

Document your assessment in the reviewer notes field.

Take Action

Choose one of three actions: Mark Reviewed, Decline, or Accept & Create Case.

Submission Statuses

When a submission email matches an existing AdoraCare member, the system automatically links the counselee to that member record. When you accept a submission, a new Case is created with the counselee pre-populated — no duplicate data entry needed.

Sharing the Intake URL

In the Submissions tab, click Copy URL to copy the public intake link to your clipboard. Share it via email, your church website, or printed materials.
The intake URL is publicly accessible. Anyone with the link can submit a form. Review all submissions carefully before accepting.
